Jeanette Morgan - BSc (Hons)

Jeanette has worked for Independent Living Solutions as a Case Manager since 2013, having qualified as a Social Worker in 2001. She studied at Plymouth University gaining her Degree in Social Policy and Social Work with DipSw. She went on to complete her Post Qualifying Award in Social Work in 2005.

Jeanette has a wealth of experience working with complex cases, has excellent assessment skills and supports person-centered care planning. Jeanette is a highly motivated autonomous practitioner-driven to provide excellent outcomes for her clients 


  • Worked with adults with an acquired brain injury (ABI) and their families.
  • Worked with adults with spinal injuries, amputees, cerebral palsy, mental health and other complex conditions.
  • Writing clear concise needs assessments.
  • Liaising with a multi-disciplinary team.
  • Liaising with care agencies to ensure the care provided is excellent.
  • Coordinating, reviewing and monitoring services to ensure they are meeting needs.
  • Experience of working in both the community and hospital settings.

Jeanette works hard to maintain positive relationships with her clients and their families by using good interpersonal skills. Jeanette has good communication skills and appreciates the importance of understanding what has been communicated. She advocates for her clients at all times, promoting and supporting choice, and when needed, ensuring best interest.

Areas of Work

Case Management with adults with complex needs. Jeanette has considerable experience working with adults with neurological conditions, traumatic brain injury and physical disabilities including amputees.

Devon and surrounding areas within a 2-hour commute.


2013 – Present Case Manager with Independent Living Solutions.

2014 – Present Social Worker with Livewell Southwest. Having been TUPE across to another organization from Plymouth City council, Jeanette found herself moved into a Community team where she has now gained additional experience of complex case management within a community setting. Working with people with various disabilities, Mental Health Conditions whilst continuing to work with Acquired Brain injury as her area of special interest. Jeanette liaises with other professionals, completes complex assessments with detailed outcome focused support plans to ensure appropriate services are promoted. She focuses on maximizing independence whilst accounting for risk.

2002 – 2014 Social Worker with Plymouth City Council. Jeanette’s career has offered her the opportunity to specialize in complex care management predominantly within a hospital setting. Jeanette has worked extensively with amputees, general medical conditions and disabilities. Jeanette worked as the specialist Social Worker on a Neurology Rehabilitations Ward for several years, managing complex caseloads and facilitating timely safe discharges. This work has necessitated the development of good links with multidisciplinary teams and various agencies.
