Overview
Sarah-Jane qualified from the Colchester Institute of Occupational Therapy, University of Essex with a 2:1 honours degree in Occupational Therapy. She has worked in NHS and private sector teams in acute and community, learning disability, palliative and forensic settings: children, adults and older people with physical, sensory, learning disabilities and mental health difficulties. She takes a proactive approach to communication and problem solving.
As a Brain Injury Case Manager for Independent Living Solutions Ltd (ILS) since April 2008, she has worked with adults with acquired brain injury, learning disabilities, older people with physical and mental health problems and children with complex conditions, such as cerebral palsy.
Recent examples include:
Close working with Neuropsychologist to devise and monitor behaviour management goals and strategies for night and day care teams.
Development of the Working File including risk assessment and involvement with moving and handling
Reviewing carers' training and arranging client specific training for carers and care teams
Setting up of communications and supervision systems for teams of carers
Liaison with teachers at school, speech and language therapists and families including setting up of communications passports for clients requiring assistive technology
Multi-disciplinary case meetings/conferences including Child in Need meetings with Local Authority
Day-to-day liaison with Solicitors/Professional Deputies regarding clients' needs
Liaison with OT, buildings manager and contractors to facilitate installation of housing adaptations.
Liaison with consular authorities to facilitate repatriation of client within EU.
Client advocacy to facilitate vocational rehabilitation activities programme
Assess needs and develop adaptations for wheelchair user to enable taking of private pilot's training course.
Design of supportive travel training programme for agoraphobic adult with learning difficulties to improve mental health and social integration.
Research of products, equipment and resources in the voluntary and private sectors
