Profile

I enjoy problem solving complex situations by applying sound clinical reasoning to maximise my client’s potential. As a dedicated and enthusiastic clinician, I strive to meet the holistic needs of everyone I work with. My excellent communication skills allow me to engage with people with communication difficulties as well as liaising effectively with professionals including healthcare and legal professionals. I have spent the majority of my career working with people with complex and varied needs.

 

 

Previous Clinical Experience

2014 – Present Social Worker with Livewell Southwest.

Having been TUPE across to another organisation from Plymouth City council, I found myself moved into a Community team where I have now gained additional experience of complex case management within a community setting. Working with people with various disabilities, Mental Health Conditions whilst continuing to work with Acquired Brain injury as an area of special interest. I liaise with other professionals, completes complex assessments with detailed outcome focused support plans to ensure appropriate services are promoted. I focus on maximizing independence whilst accounting for risk.

2002 – 2014 Social Worker with Plymouth City Council.

My career has offered me the opportunity to specialise in complex care management predominantly within a hospital setting. I have worked extensively with amputees, general medical conditions and disabilities. I worked as the specialist Social Worker on a Neurology Rehabilitations Ward for several years, managing complex caseloads and facilitating timely safe discharges. This work has necessitated the development of good links with multidisciplinary teams and various agencies.
