Sarah-Jane joined Independent Living Solutions Ltd in 2008

Since joining ILS and throughout her career, she has worked effectively in various settings in the NHS, private and charitable sectors with children and adult clients with brain injuries, learning disabilities and with physical and mental health problems. She is also an ILS Professional Mentor.

  • Case management of adults and children with an acquired brain injury and cerebral palsy

  • Liaising with multi-disciplinary team members and coordinating effective rehabilitation
  • Recommending and arranging the provision of care packages, equipment, therapies

  • Advising on and facilitating minor or major adaptations.

Sarah-Jane is skilled in developing rehabilitation programmes for adults with brain and orthopaedic injuries. Using excellent communication skills Sarah-Jane is able to coordinate rehabilitation packages and liaise with multidisciplinary teams to ensure that her client’s needs are met both efficiently and effectively.

2008 – Present Case Manager/Occupational Therapist, working with adults with traumatic brain injury and children with CP, Down’s syndrome and complex needs, assisting them to maximise their potential in education, self-care, mobility, leisure and vocational activities.

Recruiting, supervision and training of care teams for children and adults.

Developing rehabilitation programmes for adults with brain and orthopaedic injuries.

Co-ordinating adaptations and equipment in response to adult, children and family needs.

2011-2013 Close working with Neuropsychologists and other professionals in multi-disciplinary teams, to develop behaviour management and rehabilitation goals and strategies for young adult clients.

2009-2011 Assessing, progressing and completing housing adaptations through liaison with architects and builders.

Working with Speech and Language Therapists and Educators, developing communication passports, identifying appropriate assistive technology and arranging advocacy around communication of needs for children.

Adult client advocacy and assistance with vocational rehabilitation activities programme.

Relocating young adult client to live independently in the community supported by 24-hour care team.

2008-2009 Advocacy for client and client’s family with consular authorities to repatriate within EU.

Assessing and developing adaptations for wheelchair user to take private pilot's training course.

Providing supportive travel training programme for agoraphobic adult with learning difficulties to improve access to the community and social integration.
