Case Manager Profile

I am a Chartered Physiotherapist with over 7 years’ experience in the NHS, alongside established experience as a Case Manager supporting adults with complex needs. My work spans a varied caseload, requiring an adaptable, solution‑focused approach to ensure each client receives tailored support that promotes independence and quality of life.

With a special interest in neuro rehabilitation and working with TBI’s,  and other neurological conditions, I have a wide breadth of clinical knowledge. My strong communication skills support my ability to work closely alongside my clients and their families to assess their needs and establish clear, achievable goals.

I have extensive experience in managing complex cases including working as part of an MDT team, managing team dynamics, and working with agencies.

As a case manager I work with a variety of clients with complex needs and enjoy using my clinical skills to ensure the best outcomes for them.

Urgent Community Response Lead HUC 

12-month secondment working with clinicians at 111. Piloting a new service, attending regular management meetings, consolidating urgent care clinical knowledge and learning and implementing telephone triage.

Senior Physiotherapist PAH Trust 

Total of 4 years and 5 rotations in Surgical/ITU (9 months), Medical (9 months), orthopaedics (5 months), MSK (5 months), A&E (11 months). I progressed and developed management skills, taught junior staff, organized external training and on call training sessions. I supervised junior and assistant staff and managed a large complex caseload.

Senior Rehab Physiotherapist EPUT 

4 months working in elderly rehabilitation, managing new junior and assistant staff, implementing an outreach service, and running discharge meetings.

Junior Physiotherapist CUH Trust 

Rotations in Medical, MSK, stroke rehab, acute neurology and surgical. I worked with a variety of patients with complex trauma injuries in neuro and stroke. I worked with patients after lifechanging surgeries also.
