I am an experienced nurse who has worked in a variety of roles in both hospitals and the community. This has given me a strong understanding of the needs of adults and young people with disabilities, which I am able to draw on to complete thorough client assessments, make informed recommendations, and implement these effectively in my role as a case manager.
My many years of nursing experience in ICU caring for critically ill patients has also enabled me to be very aware of the trauma clients and their families experience when involved in a catastrophic event. I know the importance of both supporting clients and families as they adjust to life-changing injuries.
I have been complimented on my empathetic and sensitive approach and always aim to treat all my clients as I would wish members of my own family to be treated should they be in a similar situation.

Previous Clinical Experience

Disability Assessor, Capita – Home based
Carried out client facing disability assessments for personal independence payment applications which involved extensive report writing. Reviewed clients with severe disabilities who were unable to complete their own assessments, liaison with the involved healthcare professionals and the client’s network of support to ensure they received the benefits to which they were entitled.

Community Respiratory Nurse Specialist,
Siron care and health CIC, North Somerset
Responsible for carrying out respiratory assessments in the community and associated intervention based on longer term or acute symptoms.
October 2014 – June 2021
ICU Senior Staff Nurse, Broomfield, Hospital, Chelmsford
Responsible for nursing care for critically ill patients, many of whom had been catastrophically injured in an accident.

Clinical Lead
All of our Case Managers are supported by a member of the Clinical Lead team to provide peer review, maintain clinical standards and provide continuity of service for all clients.

Sophie joined our Clinical Lead team in 2024, following over ten years of experience as a Case Manager, supporting clients with a wide range of complex needs. Along with her background in occupational therapy, Sophie has a particular specialism in working with adults who have sustained spinal cord and brain injuries.
