I advocate for my clients at all times, promoting and supporting choice, and when needed, ensuring best interests. As a highly motivated autonomous practitioner, I am driven to provide excellent outcomes for my clients.
2014 – Present Social Worker with Livewell Southwest. Having been TUPE across to another organization from Plymouth City council, Jeanette found herself moved into a Community team where she has now gained additional experience of complex case management within a community setting. Working with people with various disabilities, Mental Health Conditions whilst continuing to work with Acquired Brain injury as her area of special interest. Jeanette liaises with other professionals, completes complex assessments with detailed outcome focused support plans to ensure appropriate services are promoted. She focuses on maximizing independence whilst accounting for risk.
2002 – 2014 Social Worker with Plymouth City Council. Jeanette’s career has offered her the opportunity to specialize in complex care management predominantly within a hospital setting. Jeanette has worked extensively with amputees, general medical conditions and disabilities. Jeanette worked as the specialist Social Worker on a Neurology Rehabilitations Ward for several years, managing complex caseloads and facilitating timely safe discharges. This work has necessitated the development of good links with multidisciplinary teams and various agencies.
