Profile
Kate is an experienced nurse who has worked in a variety of settings throughout her career. Most recently, she has supported older adults with mental health concerns and their families. She also has extensive experience in community and inpatient units, working with clients with varied conditions, many of which have had neurological presentations. This has led to a particular interest in clients with brain injuries.
Kate is skilled at developing warm relationships with clients and those involved in their care. She enjoys working with clients long term, where she can thoroughly assess their needs and problem-solve to find creative solutions. She has substantial experience working with multidisciplinary teams, liaising with professionals, and has in-depth knowledge of statutory service provision.

Community Nurse for Intensive Support – Sussex Partnership NHS Foundation Trust
- Work within Intensive Support, Older Person’s Mental Health Service, to meet the needs of people living with dementia who are in crisis
- Work closely with the multi-disciplinary team and the primary care teams to manage complex symptoms of dementia which present risk
- Work closely with Adult Social Care to advocate the needs of the individual and the family and to address any safeguarding concerns
- Respond quickly to urgent referrals, carry out assessments and create care plans under pressure
- Work creatively to find solutions, accessing community resources to provide holistic care.

- Support families in their role as carer for a person living with dementia and educate and advise family members regarding symptoms and coping strategies
- Help clients navigate the system so that they received the correct health care, social care, financial support and community support services.
- Liaise with NHS services and Adult Social Care to advocate for the person’s needs
- Provide a therapeutic relationship and safe space to discuss feelings and behaviours that are difficult to cope with.
