What is Case Management?

A client might sometimes ask, “What is case management?” The answer is that it’s about placing each person’s autonomy, aspirations, and best interests at the heart of everything we do. Our case managers work to remove barriers that may prevent individuals from living life on their own terms following a life-changing injury.

They coordinate care – including managing directly recruited support workers and care teams – as well as rehabilitation, all tailored to each person’s unique circumstances. The goal is to enable clients to live the life they choose, by working collaboratively with them, their families, and support networks.

The role of a case manager is as diverse as the individuals they support. Our case managers enable clients to achieve goals that matter to them – whether that’s returning to work, visiting festivals with friends, going camping, choosing and using adapted vehicles, independently visiting shops or the local pub, or enjoying family holidays. These are not just activities; they are expressions of identity, independence, and joy – and they reflect the deeply personal nature of what progress looks like for each client.
