Profile
Elize is an experienced Occupational Therapist, having qualified in 1998, she has gained valuable experience working with adults from 18 plus with a full range of physical and cognitive disabilities and long-term chronic conditions. She is experienced at completing holistic assessments, which include the consideration of the client’s housing, care, equipment and therapy needs.
Working as part of a multi-disciplinary team she is adept in managing rehabilitation and complex care of clients, involving multi-disciplinary rehabilitation, medical and social care teams.
As case manager she manages clients’ rehabilitation plans, liaise with the solicitors and the insurance companies, providing clear and concise written communication for their legal claims.
Elize is passionate about working closely with clients, their families, and carers, to plan and implement relevant services to help them fulfil their potential.
In addition to case management work she also works as a bank OT in an acute hospital setting, for Surrey and Sussex NHS Trust, specialising in critical care and arranging safe discharges of adult and elderly patients with complex medical and care needs. Providing recommendations regarding specialist equipment provision, rehabilitation, and community care needs.

Making recommendations for safe discharges involving care, equipment and further therapy needs.
