Whether you are a referrer, client or family member, you may have questions about Case Management and the support available following catastrophic injury. Below, we’ve answered some of the most common questions about Immediate Needs Assessments, Ongoing Case Management, referrals, support teams and finding the right Case Manager.

Case Management is a person-centred approach that supports children, young people and adults with catastrophic injury.

A Case Manager works alongside the client, their family and wider support network to assess needs, coordinate support and help ensure decisions and arrangements reflect the individual’s goals, priorities and aspirations.

At ILS Case Management, we place each client’s aspirations and best interests at the heart of everything we do, helping ensure support reflects the things that are important to them and their plans for the future.

For a more detailed explanation of our approach to Case Management, visit our What is Case Management page.

A Case Manager is responsible for assessing a client’s needs, coordinating support and helping ensure the support they receive remains aligned with their circumstances, priorities and goals.

Drawing on their clinical background and professional experience, a Case Manager works closely with the client, their family and wider support network to develop, implement and review support arrangements that reflect their individual needs.

While coordinating support is a fundamental part of the role, effective Case Management is about more than bringing services and professionals together. It is about understanding what is important to the client and helping ensure the right support is in place to pursue their goals and aspirations. A Case Manager’s role can range from bringing together the professionals involved in a client’s support and coordinating therapies, care and equipment, to helping address the practical challenges that can make a meaningful difference to everyday life.

Committed to truly person-centred practice, our Case Managers provide a bespoke approach tailored to the individual, helping ensure the right people, resources and expertise are in place as needs, priorities and aspirations evolve over time.

An Immediate Needs Assessment (INA) is often the first step in the Case Management journey following catastrophic injury.

Conducted by an experienced Case Manager with relevant clinical expertise, an INA develops a detailed understanding of the client’s circumstances, daily challenges, support network, priorities and support needs. Assessments are typically completed face to face, enabling the Case Manager to build a meaningful understanding of the client’s situation and the things that are important to them.

The assessment results in clinically reasoned recommendations designed to inform decision-making, identify support needs and help clients work towards realistic and achievable goals.

An INA often provides the foundation for future planning and recommendations for Ongoing Case Management.

Learn more about our Immediate Needs Assessments.

Following an Immediate Needs Assessment, the Case Manager will prepare a report containing clinically reasoned recommendations that reflect the client’s circumstances, priorities and identified needs.

These recommendations are designed to support decision-making and identify the support that may be required moving forward, including recommendations for Ongoing Case Management and any additional support required to meet identified needs.

There is no obligation to proceed with any recommendations made, and any next steps are considered on an individual basis.

Finding the right Case Manager is a key part of effective Case Management.

Our Case Management Services team carefully considers each client’s circumstances, support requirements and preferences alongside the Case Manager’s experience, skills, specialist expertise and location. This helps us identify the Case Managers best suited to support the client and their family.

With a nationwide team of highly qualified Case Managers, we are able to match clients with professionals who have relevant experience while also considering geography and the individual circumstances of the client.

As part of HCML, ILS Case Management also benefits from wider clinical expertise, governance structures and professional support, helping ensure clients are matched with the most appropriate professional for their needs.

You can explore our team and their areas of expertise on our Find a Case Manager page. While a CV provides information about qualifications, experience and specialist interests, the videos offer a more personal introduction and help bring our Case Managers to life.

Yes. The relationship between a client and their Case Manager is fundamental to effective Case Management.

Where helpful, we offer an informal Meet & Greet appointment prior to instruction. This gives clients and their families the opportunity to meet the proposed Case Manager, understand more about their approach and experience, and feel confident in the proposed match.

Meet & Greets are optional, provided without charge, and are intended to offer reassurance and confidence at an early stage, with no obligation to proceed.

Our Case Managers come from a range of clinical and professional backgrounds, including occupational therapy, nursing, physiotherapy and social work.

They hold recognised professional qualifications, maintain relevant professional registrations and undertake ongoing professional development to ensure their knowledge and skills remain up to date.

All ILS Case Managers are registered with BABICM and are committed to ongoing professional development. Many have also achieved BABICM Advanced Registered Practitioner status, reflecting their experience and expertise within the field of catastrophic injury Case Management.
