Profile

I have many years experience both as a Case Manager and physiotherapist working with clients with a range of neurological conditions. I pride myself on being able to consistently build good working relationships with my clients and their families. Listening to what is important to them is at the heart of my day to day work and I enjoy being able to think creatively to assist my clients in finding the most appropriate support for them. I am an organised person who is keen to provide an effective responsive and timely case management service to my clients.

Previous Clinical Experience

Care Manager, Active Care Group

Managing Complex care packages for clients with complex needs, supervising support workers, liaising with funding bodies and attending MDT meetings as required.

 

January 2022 – July 2024

Case Manager, Breakthrough Case Management

Case Manager for adults and young people with a variety of neurological conditions, including ABI. Completing INA assessments and then implementing recommendations once agreed. Liaising with clients, families, MDTS and legal teams to deliver rehabilitation goals.
