Case Management for Adults following Catastrophic Injury

For adults, our case management is shaped by a deep respect for individuality, dignity, and choice. We work collaboratively to:

• Identify and pursue meaningful goals, whether clinical, vocational, social, or personal.
• Coordinate rehabilitation, care, and therapy, tailored to evolving needs.
• Support access to statutory and private services, including benefits, housing adaptations, and specialist equipment.
• Facilitate community participation, independence, and expressions of identity.

We understand that progress is not always linear, and our role is to provide consistent, responsive support that adapts over time.
