Case Management for Young People Following Catastrophic Injury
Transition is a pivotal time — not just for the young person, but for their family too. Our approach is sensitive, empowering, and grounded in the principles of the Mental Capacity Act. We support young people to:
- Explore and express their preferences, with growing autonomy in decision-making.
- Build confidence and self-advocacy skills, supported by trusted professionals.
- Navigate changes in services, including health, education, and benefits.
- Plan for adult life, including housing, employment, and social opportunities.
We also work closely with families to help them feel secure in the transition, recognising the emotional shift that comes with loosening the reins. Our case managers facilitate open conversations, shared planning, and reassurance that the young person’s best interests remain at the heart of every decision.
