Case Management Wales
Throughout Wales, ILS Case Managers provide expert guidance to clients following catastrophic injury, combining clinical knowledge with a deep understanding of local needs and priorities.
We support clients in Cardiff, Swansea, Newport, Wrexham, Bangor, and across counties such as Carmarthenshire, Gwynedd, Pembrokeshire, Powys, Bridgend, Caerphilly, Conwy, Denbighshire, Flintshire, Monmouthshire, Neath Port Talbot, Torfaen, Rhondda Cynon Taf, Vale of Glamorgan, and the Isle of Anglesey. Each Case Manager ensures support is coordinated to manage rehabilitation, daily living, or long-term assistance effectively.
Progress varies for each client. For some, it involves rehabilitation, managing daily living, or arranging long-term support. For others, it may include enjoying hobbies, engaging with new interests, or creating positive experiences with family and friends. Support is carefully structured, responsive, and centred on each client’s priorities.
ILS Case Managers deliver responsive, client-centred support, working in partnership with clients and their families to achieve sustainable, meaningful outcomes that reflect each individual’s priorities and aspirations.
